Additionally, residents will receive three home-cooked meals daily, snacks, and therapeutic diets as needed; housekeeping and laundry services; medication management; coordination of 3rd party services; assistance with managing personal affairs; transportation to medical and personal appointments and social events; as well as interactive social and recreational activities based on individual and group interests.

CURRENT PROGRAM OFFERINGS

Southern Harbor House aims to be accessible for as many community members as possible so that we may provide the necessary support our friends and neighbors are seeking. Our primary offering is for long term residential care services where individuals move to Southern Harbor House, join our family and receive meaningful compassionate care and services that adapts to changes in an individuals need over time, extending when possible through end of life.

When space allows, short-term residents are welcomed for the same comprehensive compassionate care services. Whether coming to the island for the summer, or desiring added support before returning home following a medical event, Southern Harbor House wishes to be a part of your journey. Please note, Southern Harbor House is not a rehabilitation center, nor do we provide “re-hab” services. We offer a safe environment, assistance with activities of daily living and instrumental activities of daily living including medication management.

Open Door is a supported day service opportunity for community members to be able to have access to support for activities of daily living, as well as spend time at SHH where staff, residents and guests can interact and be a part of the participant’s social network.
